Cancellation Policy

BlueAzure Healthcare LLC – Appointment Cancellation Policy

Effective Date: August 7, 2025

At BlueAzure Healthcare LLC, we strive to provide timely and quality care to all of our clients. To ensure the best use of our resources and to respect the time of our care professionals, we ask that all clients adhere to the following cancellation policy:

1. 24-Hour Notice Requirement We require at least 24 hours’ notice for any cancellation or rescheduling of appointments. This allows us to offer the time slot to another client in need of care.

2. How to Cancel To cancel or reschedule an appointment, please use one of the following options:

Use your client portal (when available)

3. Cancellations Made Less Than 24 Hours in Advance If you cancel within 24 hours of your scheduled appointment, the decision to waive or enforce cancellation consequences will be at the sole discretion of BlueAzure Healthcare LLC.

This may include:

A cancellation fee
Deducting one visit from your monthly membership allotment (for paid members)
Marking the appointment as a no-show

4. No-Shows Failing to show up for an appointment without notice will be considered a no-show. Repeat no-shows may result in suspension or termination of services.

5. Emergency Exceptions We understand that emergencies happen. In cases of medical or family emergencies, exceptions may be made. Please notify us as soon as possible to discuss your situation.

6. Staff-Initiated Cancellations If BlueAzure Healthcare LLC must cancel an appointment for any reason, we will:

Notify you as soon as possible
Offer to reschedule at your earliest convenience
Ensure no loss of service or penalties apply

7. Agreement to Policy By scheduling services with BlueAzure Healthcare LLC, clients agree to abide by this Cancellation Policy.
